Your Right to a Good Faith Estimate
Under the No Surprises Act, patients who are self-pay or uninsured have the right to receive a Good Faith Estimate (GFE) explaining the anticipated charges for medical services.
A Good Faith Estimate helps you understand what costs to expect for your evaluation and treatment — there are no hidden fees or surprise bills.
Requesting a Good Faith Estimate
You are entitled to request a Good Faith Estimate:

We will provide a written estimate within a reasonable time frame, typically within three business days of your request or appointment scheduling.
Updates to Fees
FITLA operates as a fee-for-service practice. If any changes to service fees occur, patients will be notified in writing at least 30 days in advance, as required by law.
